FEEDBACK REPORTS

Thank you for providing detailed feedback on the promotional work you have completed for SCI Fidelity artists & projects. We can't tell you how much we appreciate all of your support and hard work!

These feedback forms are the only way to track the progress of our street teams, so your assistance in gathering the requested information is greatly appreciated. (Please review these forms prior to going out on promotional runs, so that you can take note of the requested info as your doing the work)
  You must get your feedback report(s) to us no later than THREE days prior to the show to be placed on the guest list. Please submit separate feedback reports for promotions done at concerts (passing out handbills), vs. promotions at key target locations & stores (postering, etc.).
